Prevalence of diastolic dysfunction in a Colombian population. Impact of assessment guides

Background: The presence of diastolic dysfunction is a frequent finding; however, we are not clear about its prevalence in our environment, and how it varies according to the guide used for its evaluation. Objective: To identify the prevalence and degree of diastolic dysfunction, based on the 2016 American Society of Echocardiography (ASE) and European Association of Cardiovascular Imaging (EACVI) guidelines and compare it with the 2009 guidelines. Method: A retrospective t study was carried out. Echocardiograms performed between 2018 and 2022 were evaluated. Patients over 18 years of age, with left ventricular ejection fraction ≥ 50%, were included. Results: 3,110 echocardiograms were reviewed. The average age was 55.4 ± 17 years, and 42% were men. The prevalence of diastolic dysfunction according to the 2009 guideline was 39.7% (n = 1234) and according to the 2016 guideline was 20.6% (n = 634). The presence of diastolic dysfunction was more frequent in those patients over 65 years of age, or body mass index > 30 kg/m2. Conclusions: In our population, the prevalence of diastolic dysfunction is high. The application of the 2016 ASE/EACVI guidelines, compared to those of 2009, reveals a significant variation in the classification, with a decrease in prevalence. The conduct of prospective multicenter studies should be encouraged, which allow us to demonstrate the prevalence and prognostic implication in our population.
