A 70-year-old man with acute coronary syndrome and Aslanger pattern on the admission electrocardiogram. The patient was initially diagnosed as acute myocardial infarction without ST segment elevation; however, he had acute coronary occlusion that required immediate reperfusion through primary percutaneous coronary intervention. Unfortunately, this was not performed due to a delay in diagnosis. The first peruvian report of Aslanger pattern is presented with the objective of increasing its recognition, as well as its early treatment.
